There are close to 580,000 Australians living with heart disease. Toyota is supporting the Heart Foundation by urging you to understand your risk and have a Heart Health Check.
As a valued community member, we, in partnership with the Heart Foundation, invite you to attend our dealership to take part in the COMMUNITY HEARTS PROJECT.
The Heart Foundation is conducting this exciting initiative to encourage more South Australians to visit a GP for a Heart Health Check.
Please join us on the 28th and 29th August from 9am – 5pm.
Your appointment will take approximately 30 minutes and involve having blood pressure and cholesterol measurements. The Heart Foundation will also share why it is so important to visit a GP for a heart health check.
The Community Hearts team may follow-up with you 4 to 6 weeks after your appointment to check in and see if you have visited a GP. They may also ask you for some feedback.
There is no cost to take part and your participation is completely voluntary. You are also free to withdraw from the project at any time without prejudice.
To participate in the Community Hearts Project, you need to:
- • be between 45–75 years old or 30–75 years for Aboriginal and Torres Strait Islander people
- • have NOT HAD a heart attack or stroke
- • NOT HAVE KNOWN heart disease
- • NOT currently be taking blood pressure or lipid-lowering medication
- • be able to provide informed written consent
